
We want you to know what to expect from your appointment and the necessary steps you can take to help us get to know you better.
First Appointments
Once your physician gives you a referral, give us a call. We have two offices to serve you:
- Pain Physicians of Monterey:

- The Pain Center Community Hospital (CHOMP):
23625 Holman Hwy, Monterey, CA 93942
Please call 831-759-8655 to be seen in either Monterey or Salinas.
Important: Make sure you bring any referral documentation you are given, as well as your insurance information. Once you have your appointment let your primary care doctor know when you are being seen.
Your first appointment will consist of:
- Signing the necessary forms, including privacy documentation. This allows the doctor to obtain medical information necessary to give you the best possible service. These forms give us:
- Background information to ensure your safety and the effectiveness of the appointment.
- Information on your insurance and contact information in case of an emergency.
- Your contact information.
- You may meet with a nurse or medical assistant first, who will discuss your pain concerns and gather some basic information.
- Your history will be reviewed and a complaint-specific physical exam will be performed.
- The doctor will then formulate a plan for comprehensive treatment of your pain.

Follow-Up Appointments
Follow up appointments are much less involved and are used to track your progress with the plan formulated during the initial evaluation. Procedures may be scheduled, or medications adjusted in an effort to maximize relief.
Keep in mind that we will need to see your insurance information before each appointment.
